Help-Locate this complete rear glass hatch
 
I have been trying to find this glass hatch from the Blazer in this youtube video for a while. Does anyone know anything about it? It sure would solve alot of broken hatch issues.

Not sure. I am trying to see if anyone here has knowledge of where it may have been sold. It looks like flat glass or plexi/lexan/polycarbonate.

If you make a template and take it to a good glass shop, they should be able to send it out to have a tempered piece made.

Like truckster said.
I've had many pieces made over the years, and you don't need to go to an automotive glass supplier as any reputable glass place can get it done. You can specify what level of tint you want, and make sure you get it tempered because laminated glass will crack especially with no surrounding gasket.
One note, tell them not to etch "tempered" into the glass. I had to return a full set because they had put the wording right in a visible area.

Not the same but I have a funky rear hatch top that came on a rig I acquired some time ago,
A glass craft man used a Lear canopy and combined the original and the lear
