1)For the front: If your going 5 lug just use the stock 72 rotor. if your going 6 lug use the 91 silverado stock set
For the rear: I use 91 silverado blanks and then redrill 5x5. If going 6 lug leave them stock.

3) I pay $125 plus shipping for cross drilled and slotted rotors for the front with 5x5 pattern.

Much of this info has been given to me by someone who builds and sells these kits for these trucks, this is what I have and will be installing on my truck in the next few weeks/ months. I will not disclose who has given me information as I would rather not make waves although just by posting all of this I probably already am, however anyone who digs deep enough can find this info as I did.
