There is another problem you see pretty often, where it is correct on one side and incorrect on the other side. This could easily be done on new fenders where you use a pattern to drill the holes. This is done when people drill one fender and Flip the pattern instead of transposing it to the other fender.

This truck may not be a real super (wrong paint and no upper trim) but it is a good example of the flipped pattern.
