The GMT400/400 platform trucks went to an independent front end starting in 88. Are you trying to adapt an independent front end into your 67?

As far as bolting an NV4500 to a T221, it isn't going to happen without a custom adaptor.

If you have a need for a left drop t-case find a Ford NP205 or a later chain case suited to your use.

Are you wanting a true R/V fifth wheel or a gooseneck?
Consider you driveline and braking choices carefully if the trailer is very heavy.
