The truck in the picture is most likely a Suburban that is modified into a truck. A pic of the passenger side is a dead giveaway. A Suburban has 3 doors. Most people don't take the time to build a 4th door. Lot's a of fabrication. It has been done. There were some aftermarket conversions made for the Railroad and Forestry as you mentioned. Take a look at this thread. Do a search on 4 door trucks for others.
