Your exhaust is similar to something I thought of years ago. Running the pipe out of the header and under the frame, BUT you could run the tail pipe straight out the back. You can calculate how far the rear coils springs will compress completely and route your pipe accordingly.
Of course if you have 18" wide tires, this deal may not work. Mounting the muffler body vertically can help also.
I told a friend about my idea. He built such a system on his 66 C-10, it worked nicely.
