As far as a cover, Classic Industries would be a good replacement, but he Charcoal is lighter than what you have. Black is going to be a closer match than the Charcoal. If you are wanting an exact match to what you have, I would suggest finding an upholstery shop. They can easily match your color and be able to reuse your Medallions also.
