OK I was wrong on the 3 core rad outlet being on top of the frame rail it is more to the outside of the rail but the 4 core is even further as you can see in the pic however with the 20489 lower hose attached to the lower outlet it still will not reach the water pump without MAJOR distortion. It has been suggested that the four core HD rad was not an option on six cyl trucks IDN. And before anyone says I need to rotate the hose BS tried that aint happinin the fitment get worse the hose is in a relaxed position you dont force anything to fit it either fits or it dont and this is pretty obvious this dont fit!
