I had the same issue on and off for a long time. It was such a pain that if I had the truck out I would go places where I didn't have to turn it off, or if I did I was going to be there for over an hour. I too had starters checked when cold and they worked fine.
My headers don't do anything to help the problem, but large tube factory manifolds are both hard to come by and expensive.
My solution was to use a 48 inch ground cable attached to the inside starter bolt (for a more direct ground), and a reflective Kevlar Heat shield blanket around the whole starter Assembly.
I haven't had a problem since, but I hold my breath every time I do a hot start.
