I didn't remember what application, but I ordered it through RockAuto, so I went ahead and grabbed the part number for you. It is UAC HV2211C.
One of the ends is slightly smaller than the other, so I had to really crank down on my hose clamp, but other than that, I'm very happy with it. I love being able to turn heat on or off without getting out of the truck! Great for those mornings when it's cold, but then warms up during the day
