The 5 lights are actually required due to two separate requirements, both of which are directly related to width. The two outside lights are "clearance lamps", while the 3 in the middle are "identification lamps". To be placed as high as practical. 80 inches is the cutoff if I remember correctly. Like mentioned above, you can have them on any vehicle.

My 1/2 ton Ford (I know, I know) has to have them because of having extra width.
