I think all of them were done by conversion companies. Usually the person/company that wanted the truck ordered it at the dealer and it was dropshipped to the conversion company.
I saw one without back doors in TeePee autowrecking about 20 years ago and it was cracked/rusted out around the seams where the cab had been lengthened. It was interesting but too far gone to be worth fixing due to other damage to the cab.
I'd vote for stretching the frame and putting on a regular length shortbox either fleetside or stepside.
