I'm not into the whole lifted 2WD thing but I have a few ideas that may or may not work.

First, when trying to get as low as possible, guys are notching their crossmembers to drop the frame into the front suspension. Why not try to do a spacer of some sort to lift the frame out of the crossmember. I know it wouldn't be quite that easy, but it's an idea.

Second, I read , I believe in the FAQ section, that for a cheaper disc drake conversion for the 67-70 2WD, you could use (I believe) a 1970 3/4 ton lower control arm and your stock upper control arm to mount the balljoints for a 88 and up chevy truck. If this is true then you should be able to use a lift spindle for those model trucks..., correct?

Again, I have NO idea if this will actually work, just trying to help you guys out with a fresh idea that could maybe be taken further with a little more thought.
