When I bought my 1969 new, I asked the salesman where the inside mirror was. He responded that on the '69s an inside mirror was an option until later in the '69 model year. This is verified based on the 1969 Chevy truck "info kit" (
https://www.gm.com/heritage/archive/...formation-kits) which contains info regarding included and optional equipment. The option code was D36.
Also, there were no pre-drilled holes for a mirror. I believe the mounting bracket shown above was actually for a '67.
