You mean you guys haven't done this to your trucks yet...? LOL Honestly that guy must have some kind of "special" relationship with that box, thats the only conclusion I can come up with as to why someone would do this to that truck.

I think 1970 CST short wide is right. It looks like it is trying to cross breed. Kind of like a german shephard and a chihuahua (if that's how you spell it).
