Busted throwout bearing cause?

Been driving sticks my whole life, I don't even own an automatic.
I don't ride the clutch.
So, what caused this?
Bad luck? improper adjustment? (it seemed adjusted properly)
I installed this bearing with a new clutch pack about 10,000 miles ago.

Made in china bearing that locked up?? that's a new one on me also, never seen one do that. Nose cone of the trans ok?

Yes, the trans is ok.
Rhino-pac clutch pack. I thought it was made in Mexico.

Rhino Pak clutch kits are warranted for 12 months, 12000 miles....

The clutch fork will be toast...

Try this one....I have had good results with it....

If it needs a kit....open the kit before you buy it and check country of origin of all parts....stay away from Chinesium and Mexican stuff...dubious quality...

The clutch fork is still in good condition, It was replaced when this clutch was installed and I tore it down almost immediately after it failed.
No damage.

That's not the first one that I have seen do that. I have a McCloud adjustable bearing in the truck that did the same thing. Luckily I can just push it all back together until I get around to pulling it out.
