TH350 Decoded?

I have a TH350 that I was trying to figure out what it came out of originally. The numbers I found on the side of transmission were 12y150207 and also another set of numbers 21905211797. After research online I think ive figured out that 1 means Chevy, 2 means 1972, y means Toledo manufacture and the last 6 numbers mean car it came out of but cant determine what car it is. Any help would be greatly appreciated.

The last 6 numbers in the first sequence you posted are the last 6 numbers of the car it was in's VIN (partial VIN like they stamped on engine blocks). So that isn't going to tell you what car it came out of.

I don't know what the second sequence means.
