correct intake manifold - where?

'71 C10, with a 350 out of a '75 truck... Our curse continues trying to right all the wrongs of a machine shop/rebuilder here in AZ that knew nothing and tried to hide it.

We went to put on a new thermostat housing because they had some screwy bolt in there, with 5 washers to keep it tight. Then we realized why. They had broken off the original bolt in the manifold, and tapped it... off center, too short, and for a smaller bolt.

We could maybe retap it correctly, but I think I just want a clean slate and get a new or good used OEM-style intake manifold. I'm just not sure where to find what we need. We want one with the two holes for the original style divorced choke (which we also have to buy because the shop never put one on.)
