Help ID This Rear End?

I have had this for about 20 years, I it is Chevrolet and I am assuming from somewhere between 1916 and 1928. I have been trying to figure out the exact application.

I have been told the car rear ends have 4-bolt hubs and 4-bolt rims and the trucks used 6-bolt hubs and rims. This one has 6-bolt hubs and 4-bolt rims. Does anyone here have any experience with these and possibly help me out with some info?
