I went to college with a guy that had been a reporter for a newspaper. He said the paper once had a picture of a group of guys and named each one. The night before a typesetter didn't have the name for one of the guys in the picture so just put "some fu**er" as a joke but forgot to fix it.
The paper printed "left to right: Jeff X, Brian, Y, Some Fu**er, Lisa M."
