You Got lucky finding that switch, they're hard to come by these days. That is from a 1981 van or 81-83 truck.
Blue- Ignition 12v
Red- Set/Coast
Yellow- Resume/Accel
Green- On/Off

A million thanks....I've looked everywhere but wasn't sure what it came from....
another piece to the puzzle....

one last question, will this switch need relays or will it wire directly to the CC module?

No relays needed with that switch since it's a GM switch
