rear disk brakes

I have a 69 c10 with a 1971 c10 rear installed in my truck to convert to 5 lug. I would lie to convert to disk brakes does anyone have a suggestion on what to use and where to buy? I would like to buy a setup that will take stock replacement parts from a local parts store if I have a repair problem.: :

does astro van rotors from 1992-02 fit on the rear axle from my 71 rear wihout modification. the inside hole is 3" and the hub with the lugs
is 6 1/2" ?
