Mounting DG4 Below Eye Line Mirrors

I had intended to wait to mount my DG4 mirrors until I had refurbed the doors. But daily driving in SF Bay traffic has made me paranoid about my lack of functional mirrors (stupid sport mirrors are too damn small and passenger side won't adjust far enough in).

So, I'm looking for advice on mounting DG4 mirrors on doors that aren't drilled for them. Hole pattern, backing, best mounting screws/bolts, any advice is helpful.
