I have a Derale 18916, very similar fan.

Your PCM is capable of controlling the fans, but I don't think in an '04 would have that programmed so you would need to have it added when you do the reflash.

The fan is controlled using a ground trigger, so you will need to use a relay - the PCM grounds the coil on the relay to turn it on, and that's what activates the fan. There's a single wire you have to add to the harness coming from the PCM for this function, if the donor truck didn't have E-fan to start with.

That shows the pin-outs for the 2004 PCM - the wire in question goes to pin 42 in the blue connector C1 (the pin is highlighted in blue as well).

I can't remember, were you going to modify the harness yourself, or buy one of the modified ones?
