Clutch won't stay adjusted

My 83 C20 granny 4 spd clutch will not stay adjusted. Is it normal to have about 1/8" of play between the swivel on the adjustment pushrod and the hole in the end of the bell crank? This causes the swivel to rack/twist in the end of the bell crank when the clutch is engaged/disengaged, and I think this may be causing the stop/lock nut mechanism to work loose. It seems to me like this setup should have some sort of bushing inside the hole of the bell crank to take up the slack, but I don't see any kind of bushing in the catalogs I have seen. If this play is not normal, how do I determine if I need a new bell crank or swivel to take up the slack? Is there any specification on the allowable inside diameter on the end of the bell crank hole, or the outside diameter of the swivel? Could either of these two parts have worn too much in a mere 27 years? I took the whole lower pushrod assembly out of the truck and the stop/lock nut mechanism works fine - locks up tight very easily. Thanks.
